Learning the Lessons from Serious Case Reviews: the Role of Practitioners

The Practitioners Forum evolved through the organisational learning that took place following two local Serious Case Reviews. As briefings were held across the partnership, a determination to ensure that the lessons learnt remained alive and became embedded into local practice and the Forum took shape. Practitioners attending the briefings wanted to be part of this process and with a strong commitment from the LSCB the Practitioners Forum was born.

The Forum is multi-agency and has representation from local services that are working with children and families. The work of the Forum contributes to and compliments the safeguarding agenda.

The aims

The Practitioners Forum aims to provide a regular feedback loop and reality check from practitioners to the LSCB and senior management about how well, or not SCR lessons and action plans are being implemented.
It also identifies ideas for improvement so that changes can be made systemically.
